If you experience any difficulties completing the referral form, please contact us via email at referral.ngs@nhs.net providing a contact number, or call us on 0300 300 1490 giving a contact number if leaving a message.

Upon receipt of the referral, we aim to contact the service user with next steps within one to three days.
